Member Duties : The ICMS Resource Coordinator VISTA will serve in the Community Wellness Clinic. The VISTA is tasked with leveraging resources surrounding ICMS (Intensive Case Management Services) clients and developing more effective pathways to healthcare resources for this population. Specifically, you will develop more efficient ways to link these clients to primary care, behavioral health care, specialist, and dental care. We believe that this role is pivotal to providing care for clinic clients by ensuring healthcare systems and delivery methods remain current, evidence-based, and responsive to the needs of the unhoused community.
